COOPER'S HOME CARE
A holistic, hands-on approach to nursing care where we treat the whole person, not just a particular problem or a group of symptoms. We encourage family participation in the assessment and identifying/needs phase before and after admission. Our team of professional care providers realizes the importance of family involvement, communication and feedback. Our staff collects data of prior lifestyles, likes and dislikes and a variety of behaviors and health issues of what the resident was like prior to their senior life changes. This information about the resident provides workable tools that help form and stabilize the framework for care planning.
The care planning team continues to assess the resident and identify problems and needs with supporting evidence or behaviors. A plan of action is written with an outline of the expected outcome of those actions. The resident plan of care is sent to the resident physician for review, any necessary changes and signature. The plan of care is placed on the resident’s chart. There is an ongoing assessment with revised interventions as needed to assure the stated goals and probable outcome is achieved.
